Cook Plant Manager, Douglas Cooper, resigns. Mr. Douglas Cooper, Plant Manager for Donald C. Cook Nuclear Plant, Units 1 & 2 has resigned to pursue other career opportunities. Mr. Cooper has been the Plant Manager at D. C. Cook since January 1998. His last official day will be June 11, 1999. Mr. Chris Bakken, Site Vice President, will be acting Plant Manager until a replacement has been found. Regional Action: None. Contact: A. Vegel (630)829-9620 _ REGION IV MORNING REPORT PAGE 2 JUNE 2, 1999 Licensee/Facility: Notification: MR Number: 4-99-0017 Fansteel, Inc. Date: 06/01/99 Muskogee,Oklahoma License No: SMB-911 Subject: TORNADO AND THUNDERSTORM DAMAGE AT FANSTEEL SITE IN MUSKOGEE, OK Discussion: On June 1, 1999, around 7:00 p.m. (CDT), the Fansteel, Inc. site near Muskogee, Oklahoma was damaged by a tornado and thunderstorms. Licensee representatives stated that the facility was not operating at the time of the storm. According to the licensee's damage assessment, the site lost power, telephone service, and several facility buildings were damaged. The facility backup diesel power generator started automatically as designed. The Sodium Reduction Building, which is the main storage facility of packaged radioactive material (RAM), was affected, with metal siding and part of the roof blown off. At least four one-ton bags of RAM fell outside of the building because of the storm and a collapsed wall. However, a licensee representative stated that the bags were not breached and no RAM spilled on the ground. There were no chemical releases or storage tank damage. The personnel change house and security guardhouse were destroyed. The main and secondary process buildings (Chem-A and Chem-C) and the site office building sustained roof and siding damage. The groundwater cleanup system sustained some damage in that two pump/sample station huts were missing and two doors were blown off the groundwater evaporation building. Additionally, the environmental and effluent monitoring stations were inoperable due to lost power. Although the monitoring stations were inoperable for some period of time, no further damage to the groundwater cleanup system has been identified at this time. According to Fansteel management, the facility will be remain shutdown for the next two to four weeks. The licensee is continuing its investigation of the extent of damage, and Region IV plans to get updated information later today. This information has been discussed with the State of Oklahoma.
