STUCK OPEN SHUTTER
The following information was provided by the licensee via phone:
On November 1, 2025, at 1300 MDT, a normally open, vessel mounted gauge was found stuck in the open position during a periodic inspection. The manufacturer has been contacted to schedule replacement of the gauge. There is no risk of additional exposure to personnel due to this event.
