AGREEMENT STATE REPORT - STUCK SHUTTER
The following report was received via email by the Iowa Department of Public Health:
"A fixed gauge [Isotope Measuring Systems, Inc., Model Number 5221-02, with an Am-241 5 Ci source] was given indications of improper function. When workers entered with the survey meter, they identified radiation levels were elevated, indicating a stuck shutter. The gauge and gauge assembly were rolled into an isolated and secured in a storage facility. Their service provider will be on scene tomorrow [8/18/22] to assess the fixed gauge. Initial cause is a partial closing of the shutter due to a faulty screw. The licensee in pursuing replacement of fixed gauges due to the age of the devices."

AGREEMENT STATE REPORT - UNABLE TO RETRACT SOURCE
The following report was received via email from the Texas Department of State Health Services [the Agency]:
"On August 18, 2022, at 0922 [CDT] hours, the Agency was notified of a source retrieval that occurred August 17th, 2022. The licensee stated the event occurred at a field location at 1245 [CDT] on August 17th, 2022. The licensee stated the location was at a facility located in Pampa, Texas. The licensee stated the event occurred when a piece of pipe fell onto the guide tube of a SPEC [Source Production and Equipment Company] - 150 exposure device and crimped the tube, preventing them from retracting the 60 Curie iridium [Ir-192] source. The licensee's retrieval individual was able to reshape the guide tube and the source was successfully retrieved by 1330 [CDT] that day. The exposure to the individual retrieving the source was 80 millirem. The associated equipment was removed from service and is undergoing inspection to ensure no other damage was done to the equipment. Additional information will be provided as it is received in accordance with SA-300."

AGREEMENT STATE REPORT - BROKEN WINDOW ON NUCLEAR GAUGE
The following information was provided by the State of South Carolina, Department of Health & Environmental Control (DHEC) via email:
"On August 18, 2022, at 0948 EDT, DHEC inspector Leland Cave received a call from Mitsubishi Polyester Films, LLC (S.C. Lic. No. 036) to report an incident that had occurred at the plant. The licensee stated that earlier that morning the thin window film was torn on one of their Thermo EGS Gauging, LLC beta gauges [1.25 Ci of Kr-85]. The result of the tear was that the production line was stopped until the film could be replaced. The representative from Thermo was already onsite and needed verbal permission to begin the repair on the unit. The permission was given, and the reciprocity notification was sent in.
"The DHEC inspector called in the incident [to the NRC] at 0936 EDT on August 19, 2022, and is preparing to go to the licensee to review the cause of the incident."
