Subsequently, these | | sprinkler systems were declared inoperable, and TSR-required actions | | establishing roving fire patrols were initiated. This deficiency was | | detected during scheduled system inspections conducted by Fire Protection | | personnel. The one | | head on system B-8 was in the area already being patrolled. This condition was discovered as a result of an | | engineering review of individual sprinkler heads that had previously been | | identified as exhibiting corrosion around the valve seat. The engineering | | review was conducted following the discovery that similar corrosion problems | | experienced at the Paducah Gaseous Diffusion Plant resulted in sprinkler | | heads being unable to actuate at normal pressure. An initial screening of | | PORTS sprinkler heads known to exhibit corrosion was conducted, and systems | | where the potential for two or more adjacent heads to be corroded were | | inspected. This identified 11 systems in the X-333 and 2 systems in the | | X-326 that did not meet the sprinkler system operability requirement that no | | two adjacent sprinkler heads be inoperable." | | | | "The PSS declared the affected sprinkler systems inoperable, and TSR | | required actions were completed. Further inspections of sprinkler systems | | are planned. The licensee reported that the annunciators are currently | | inoperable, and that additional panel walkdowns are being performed. The | | unit is currently stable in Hot Standby following a reactor trip earlier | | today (see EN 36104). The Charlevoix | | Township fire department responded to site, but the on site fire brigade was | | able to take care of the problem. The problem was due to main steam pipe | | cutting activities heating the pipe insulation cloth coverings to the point | | of generating embers. The out of tolerance condition was in a non-conservative | | direction and previously discovered during a routine weekly surveillance on | | 8/27/99. An evaluation to determine the condition existed while the MFS was | | in operation was initiated at that time. Engineering suspected that the | | probe calibration may have been affected when the probes were exposed to a | | high pH solution on 8/23/99. Subsequent discussion with the probe | | manufacturer confirmed that a high pH solution could affect probe | | calibration. As a result, it was concluded that the out of tolerance | | condition likely existed from approximately 8/23/99 to 8/27/99. | | | | "The out of tolerance condition affected 2 of the 3 pH probes (2 out of 3 | | logic required to activate the safety system). The as found data for the 2 | | probes indicated they would actuate at 6.73 pH and 6.65 pH, which is below | | the 7 pH required setpoint. To complete this change, | | SCE installed a temporary panel in the control room area and removed the | | original panels and some cable wraps. On August 31,1999, shortly before | | midnight, licensee personnel noticed that several cables for Train A, Train | | B, and Train X (e.g., various low voltage electrical power cables and | | instrument signal lines) appeared to not meet the separation criteria of | | Regulatory Guide 1.75, 'Physical Independence of Electrical Systems.' | | Several of the affected cables provide actuation for Engineered Safety | | Features (e.g., control room isolation and containment purge isolation). The | | Updated Final Safety Analysis Report for Units 2 and 3 indicates these | | systems comply with IEEE-297, 1971, 'Criteria for Protection System for | | Nuclear Power Generating Stations' which is referenced by Regulatory Guide | | 1.75. Consequently, at 1526 PDT, SCE concluded this condition was reportable | | in accordance with 10CFR50.72(b)(1)(ii)(B) as a condition 'outside the | | design basis of the plant'. | | | | "Shortly after identifying this condition, control room operators (1) placed | | Train A of the Control Room Emergency Air Cleanup System in service, and (2) | | closed and removed power from the containment minipurge valves. SCE | | determined that this condition has only existed since August 27, 1999. SCE | | has also confirmed that the affected components remained capable of | | performing their intended safety functions and, therefore concludes the | | safety significance of this occurrence is minimal." | | | | The NRC resident inspector has been informed of this notification by the | | licensee. | +------------------------------------------------------------------------------+
